Scar Therapy
By far the most attractive outerwear for this season and any season is perfect skin. It never goes out of style. We are born with it and it is flawless. But time and sun take it toll as do life’s little traumas. So with time we end up with scars.
A single scar isn’t so bad especially if it is small or hidden but scars can also be embarrassing and bothersome. Scar formation is the result of the skins natural healing process. How your skin heals after an injury depends on several factors:
First of all the actual trauma itself i.e. whether it was an abrasion or a clean cut. Repair depends on the tension on the wound during the healing time and the depth of the injury. Your age and ethnicity also play a role. Finally how you treat the newly formed scar is the one factor that you have control of and will affect the outcome of the scar formation.
Let’s review the types of scars and then discuss how to treat them:

The normal everyday scar: This results from the dermis being damaged or cut. It can result from an ulcerative injury, a cut, an abrasion or even inflammation alone (as in acne). These are the scars we can work with the most with topical creams. The old standby has been to treat the scars with silicone. You can use topical sheets, topical gels or silicone products. Silicone keeps a protective coat on the scar and allows the wound to heal in a moist environment.
- Keloid scars: These are the rarest of scars but the most debilitating. These are scars that extend over and above the original injury. These can often be five times as large as the original injury. They protrude outward and are more prevalent in people of color. Possible treatments include surgical excision with post excision pressure applied and frequent steroid injections. Steroid treatment alone with pressure may also prove to be of some benefit. For this condition it is best to use the silicone sheeting.
- Hypertrophic scars: These scars are often used interchangeably with the term Keloid but they are not the same. Hypertrophic scars do not extend beyond the injury at the borders. Rather they are raised and may project slightly outward but they stay within the boundaries of the scar. These are treated with steroids and pressure as well. Intralesional Steroid injections with silicone sheeting will help with this.
Keloids and hypertrophic scaring are characterized by an accumulation of excess collagen and are distinguished from each other by their appearance. Keloids grow beyond the borders of the original wound and do not tend to resolve spontaneously. Hypertrophic scars stay within the limit of the original wound and do tend to regress spontaneously. Hypertrophic scars are generally seen soon after tissue injury, whereas keloids can form as late as a year after injury. Hypertrophic scars tend to be associated with a contracture across a joint surface

Hyperpigmented scars: These are scars that take on a darker color
after they have healed. Generally a scar left untreated will eventually return
to the normal color of the skin but this could take 6 months to years later.

Hydroquinone prevents the overproduction of melanin from occurring, while
lightening the brown spots on the skin.
Hypopigmented scars: These are scars that heal without pigment. They are a lighter color than the surrounding skin. While a laser has been introduced called XEmere, its results have been only temporary at returning the skin's natural pigment. The scars will not return to normal pigment if left untreated.
